It’s interesting to note that the names in this record are spelled differently than they are spelled in other records .  In this record, the surname is spelled Izbiski, but in later records, the surname is spelled Izbicki .  In this record, Bernard’s father is Boleslaw, but in later records his name is spelled Boleslas .  In this record, Bernard’s mother is Victoria Krutzinski, but in later records her name is spelled Victoria Kruczynska.

Bernard’s Death Certificate shows that the place of origin of his parents is Poland .  His Birth Record is a bit more specific – it states that Bernard’s parents were born in Russia-Poland.

In most of these earlier Vital Records, there are two page numbers listed – one written at the top center of the page .  The one that is actually used to find the records is stamped in the upper left.

The actual image for this record was obtained from the high quality images on the website of the New England Historic Genealogical Society .  These images are just one reason why those with New England Ancestors should join the NEHGS.

There are several stray marks on Anastasia Rita Meleski’s application, including the initials CL at the top, the initial M in two places at the bottom, the numbers 420 and 21 to the right, and the notation “Park View Rest.” on the left.

The way Anastasia Rita Meleski initially entered her name on the application was A. RITA MELESKI, indicating that she may have routinely used the name Rita instead of Anastasia at this point in her life.

The name Anastasia Rita provided for her mother was U. Katherine Warchol . This is the first time I’ve seen the letter “U” used in Katherine Warchol’s name .  Perhaps her first name is not Katherine after all . The Big Book of Names provides a number of choices for Polish first names beginning with “U” . The most common I’ve seen is Urszula . I’ll have to keep this in mind if I look for the Birth Record for U. Katherine Warchol.

The Death of Albert F. Blackman

Anastasia Rita Meleski’s first husband, Albert F. Blackman, died on 11 Jul 1966 .  The Certificate of Death for Albert Blackman is not the Standard Certificate of Death .  It is, rather, a Medical Examiner’s Certificate of Death .  I’m not sure what circumstances required a Medical Examiner’s Certificate .  Perhaps the fact that Albert was a relatively young, 51-year-old man when he died prompted the investigation.

  • Albert F. Blackman, a 51-year-old white male, married to Rita Meleski, died on 11 Jul 1966 at 147 Cullinane Drive, Marlborough, Middlesex County, Massachusetts;
  • Albert was a veteran of World War II and had resided at 147 Cullinane Drive for 10 years;
  • Albert’s usual occupation was as a writer for the Record American; he apparently did not have a Social Security Number;
  • He was born in Roxbury, Massachusetts; his father was Clifford Blackman who was born in Boston, Massachusetts; his mother was Jane Goodrich who was born in Charlestown, Massachusetts;
  • The informant was his wife, Mrs. Rita Blackman of 147 Cullinane Drive, Marlboro, Massachusetts;
  • The cause and manner of Albert’s death was heart disease, presumably coronary sclerosis – he was found dead in bed;
  • Albert’s death was not an accident, suicide, or homicide, no autopsy was performed, and his death was not related to his occupation;
  • The death was certified by R. N. Rittenhouse, M.D. of 540 Bolton St., Marlboro, Massachusetts on 11 Jul 1966;
  • Albert was buried in Notre Dame Cemetery, Worcester, Massachusetts on 14 Jul 1966 by John J. Brown & Son of 95 West Main Street, Marlboro, Massachusetts;
  • The certificate was filed on 12 Jul 1966 by Rita T. Lapine and the burial permit was issued on 12 Jul 1966 by Peter P. Cottone, an Agent for the Board of Health;
  • The Certificate of Death was recorded in Volume 72, Page 35 of the 1966 Death Certificates of Marlboro June 2 – Dec 30 to Medford Jan 1 – Mar 31;
  • Miscellaneous other numbers on the Certificate include a handwritten 9-28 and a stamped number that appears to be 028935; the word MEDICAL appears in large print across the face of the document.

Albert died very young .  It’s no wonder that Anastasia Rita remarried two years later.

The certificate states that Albert’s mother, Jane Goodrich, was born in Charlestown, Massachusetts, but other documents, including Albert and Anastasia Rita’s Marriage Certificate and Jane Goodrich’s Birth Record, state that Jane was born in Everett.

One curious statement on the Death Certificate form itself is that the Agent of the Board of Health certified that a satisfactory STANDARD Certificate of Death was filed .  In fact, a Standard Certificate of Death was not filed – a Medical Examiner’s Certificate of Death was filed instead.

My half first cousin once removed, Anastasia Rita Meleski, was married twice .  Her first marriage was to Albert Francis Blackman, who predeceased her .  In the record of her marriage to Bernard J. Izbicki, her name is listed as Rita (Meleski) Blackman.

  • She was a social worker, and her parents were Joseph Meleski and Katherine Warchol
  • Bernard was divorced, Rita was widowed, and this was the second marriage for both of them
  • The intention of marriage was entered at Marlborough on 17 May 1968 and the certificate was issued on 24 May 1968 by Rita T. Lapine
  • The wedding was solemnized on 04 Jun 1968 in Our Lady of Czestochowa Church in Worcester by the Right Reverend Charles J. Chwalek, a priest residing at 34 Ward Street, Worcester, Massachusetts
  • The certificate was received by the city or town clerk, Rita T. Lapine on 06 Jun 1968
  • The Registered No. was 90 and the Intention No. was 86
  • There is a notation that “Meds on file” (probably the medical certificate of death for Rita’s first husband) and that Divorce papers on file” (Bernard’s divorce)
  • The certificate also notes the page number 311 in Volume 49 of the Marriage Records for Marlborough in 1968, and further there is a hand written notation 14-61

The record shows that Rita was still living in the home that she and Albert Blackman shared before his death two years earlier.

The most interesting part of this marriage record, however, is that even though Bernard Izbicki was divorced, this marriage took place in a Roman Catholic Church .  The only way that I think this could have happened is if Bernard’s first marriage was not solemnized in the Catholic Church .  Very unusual .  I’ll have to look for the record of Bernard’s first marriage and the record of his divorce .  Very unusual, indeed.

Some Genealogical Resolutions for the New Year

The year 2006 is rapidly reaching its end, and with the advent of the New Year it’s a good time to evaluate my genealogical research and prepare a research plan for the next twelve months .  In no particular order, other than the order in which these thoughts come to mind, here are my genealogical resolutions for the New Year:

  • Complete the coursework for a Professional Learning Certificate in Genealogical Studies (PLCGS) through the National Institute for Genealogical Studies / University of Toronto
  • Hire a professional genealogist in Poland to visit Dubiecko and Dylągowa parishes to help trace my Dańko and Dziurzyński ancestors there (these records have not yet been microfilmed by the Family History Library)
  • Renew my lapsed membership with the California Genealogical Society and volunteer to help with one of their projects (this group has transcribed an incredible number of records from the San Francisco Bay area)
  • Finish the transcription of Polish village names in powiat makowski for the Polish Genealogical Society of America
  • Record oral histories from my relatives, a big project that I think is best undertaken a little at a time, but one that I need to begin
  • Finish reading The Peasants by Wladysław Reymont
  • Attend a major national genealogical conference or genealogy cruise
  • Clear out “The Box” – this pile of genealogical papers has become so large that I’m convinced they’re reproducing on their own
  • Publish an updated family history for at least one of my family lines and distribute copies to my relatives
  • Begin a one-name study of the Niedziałkowski surname

Whew!  I’m exhausted just thinking about all this!  I may not accomplish all of these goals in 2007, but that’s OK, too .  A little at a time, bit by bit, record by record, the story of my ancestors unfolds.
