This happened in Zielona on the third day of April in the year one-thousand eight-hundred sixty-one at the hour of seven in the morning . There appeared Daniel Grabowski, fifty-seven years of age, and Józef Budiński, twenty-seven years of age, both residing in Rączki, and they stated that on the first day of this same month Jan Chodkowski died, nine weeks old, son of the married couple Wojciech and Joanny Chodkowski in Rączki. After visual confirmation of the death of Chodkowski, this document was read aloud to the declarant and was signed by Us. The declarant does not know how to write.
