He was baptized in the parish church in Płoniawy, Maków Powiat, Płock Gubernia, Congress Kingdom of Poland on 11 September 1836.

â„– 64 Chodkowo wielkie
This happened in the village of Płoniawy on the thirtieth day of August / eleventh day of September in the year one-thousand eight-hundred thirty-six at the hour of eleven before noon. There appeared the noble and well-born Jan Chodkowski, owner of a part [of the village] and residing in Chodkowo wielkie, forty years of age, together with Jan Chodkowski, sixty years of age, owner of a part [of the village] and residing in Chodkowo wielkie, and he showed Us a child of the male sex born in Chodkowo wielkie on the twenty-eighth day of August / ninth day of September in the same year at the hour of eight in the evening of his wife Joanna née Kobylińska, forty-six years of age. At Holy Baptism performed on this day, this child was given the names Mikołaj Andrzej, and the Godparents were the married couple Andrzej and Anna Chodkowski. This document was read aloud to the declarants and witnesses who do not know how to write and was signed by Us.
