Stefan Chodkowski, son of Melchior Chodkowski and Kunegunda Zbyszyńska, died on 28 November 1839 in Chodkowo kuchny, Maków Powiat, Płock Gubernia, Congress Kingdom of Poland. He was buried in Płoniawy, Maków Powiat, Płock Gubernia, Congress Kingdom of Poland on 29 November 1839.

This happened in the village of Płoniawy on the seventeenth / twenty-ninth day of November in the year one-thousand eight-hundred thirty-nine at the hour of ten in the morning. There appeared Melchior Chodkowski, father of the deceased, seventy years of age, and Leon Chodkowski, sixty-seven years of age, owners of parts [of the village] and residing in Chodkowo kuchny, and he declared that yesterday at the hour of nine in the morning, Stefan Chodkowski, owner of a part [of the village] and residing in Chodkowo kuchny, died in Chodkowo kuchny, leaving behind his widowed wife, Anna née Roman. After visual confirmation of the death of Stefan Chodkowski, this document was read aloud to the declarants who do not know how to write and signed by Us.
