Klementyna Chodkowska, daughter of Kacper Chodkowski and Aleksandra Milewska, was born on 26 April 1863 in Umiemy, Ciechanów Powiat, Płock Gubernia, Congress Kingdom of Poland. She was baptized in the parish church in Zielona, Ciechanów Powiat, Płock Gubernia, Congress Kingdom of Poland on 29 April 1863.

No 41 Umiemy This happened in Zielona on the twenty-ninth day of April in the year one-thousand eight-hundred sixty-three at the hour of four in the afternoon. There appeared Kacper Chodkowski, owner of a part [of the village], thirty-three years of age and residing in Umiemy, in the presence of Stanisław Kołakowski, forty years of age, and Franciszek Sumiński, thirty-two years of age, residing in Umiemy, and he showed Us a child of the female sex born on the twenty-sixth day of this same month and year at the hour of seven in the afternoon of his wife Aleksandra née Milewska, twenty-four years of age. To this child at Holy Baptism performed on this day was given the name Klementyna, and her Godparents were Edward Chrzanowski and Klara his wife. This document was read aloud to the declarants, the father, and the witnesses and was signed by Us and the declaring father; the witnesses do not know how to write.
