Jan Chodkowski, son of Leon Chodkowski and Konstancja Milewska, was born on 13 December 1941 in Mosaki Rukle, Przasnysz Obwód, Płock Gubernia, Congress Kingdom of Poland. He was baptized in the parish church in Krasne, Przasnysz Obwód, Płock Gubernia, Congress Kingdom of Poland on 14 December 1841.

64 Mosaki Rukle This happened in the village of Krasne on the second / fourteenth day of December in the year one-thousand eight-hundred forty-one at the hour of two in the afternoon. There appeared Leon Chodkowski, owner of a noble part [of the village], twenty-six years of age and residing in the village of Mosaki Rukle, in the presence of Franciszek Niedziałkowski, twenty-six years of age, and Jakób Chodkowski, twenty-six years of age, both owners of noble parts [of the village] and residing in the village of Mosaki Rukle, and he showed Us a child of the male sex born in the village of Mosaki Rukle yesterday at the hour of twelve noon of his wife Konstancja née Milewska, twenty years of age. To this child at Holy Baptism performed on this day was given the name Jan, and his Godparents were Franciszek Nisiobędzki. This document was read aloud to the declarants and witnesses and signed only by Us because the people mentioned in the document do not know how to write.
