No 13Â Mosaki Rukle
In the year one-thousand eight-hundred eighteen on the second day of the month of August at the hour of eleven before noon, before Us, the Reverend Mikołaj Ruciński, Registrar of the Civil State of the Krasne Gmina in the Płock Województwo, Przasnysz Obwód and Municipality, there appeared the well-born Jan Chodkowski, father of the deceased, forty years of age, and Walenty Niedziałkowski, an unrelated neighbor, thirty-six years of age, both landowners residing in Mosaki Rukle, and they declared to Us that on the first day of the month of August in this same year at the hour of nine before noon, the well-born Jakub Chodkowski, one week old and son of the well-born Jan and Marianna née Obidzińska Chodkowski residing in Mosaki Rukle, died in the home of her parents under the number eleven. Whereupon, since none of the declarants know how to write, this document was signed by Us, the Registrar of the Civil State, after reading it aloud to them.
