Michał Roch Chodkowski was born on 16 August 1841 in Chodkowo Załogi, Pułtusk Obwód, Płock Gubernia, Congress Kingdom of Poland and was baptized on 22 August 1841 in Płoniawy, Pułtusk Obwód, Płock Gubernia, Congress Kingdom of Poland. He was the son of Piotr Marek Chodkowski and Jadwiga Suwińska.

This happened in the village of Płoniawy on the tenth / twenty-second day of August in the year one-thousand eight-hundred forty-one at the hour of ten before noon. The noble Piotr Chodkowski appeared, a landowner residing in Chodkowo Załogi, forty-eight years old, accompanied by Teodor Chodkowski, forty-four years old, together with Tomasz Chodkowski, twenty-five years old, and he showed us a male child born in Chodkowo Załogi on the fourth / sixteenth day of the same month and year at the hour of eleven at night of his wife Jadwiga née Suwińska, thirty years old. To this child at Holy Baptism performed on this day was given the name Michał Roch, and his Godparents were the married couple Teodor and Katarzyna Chodkowski. This document was read aloud to the declarants and witnesses who do not know how to write and signed by us.
