Zuzanna Zofia Chodkowska, daughter of Stefan Chodkowski and Anna Roman, was born on 20 September 1839 in Chodkowo kuchny, Maków Okręg, Płock Gubernia, Congress Kingdom of Poland. She was baptized in the parish church in Płoniawy, Maków Okręg, Płock Gubernia, Congress Kingdom of Poland on 22 September 1839.

Chodkowo kuchny 62.
This happened in the village of Płoniawy on the tenth/twenty-second day of September in the year one-thousand eight-hundred thirty-nine at the hour of eleven before noon. There appeared the well-born Stefan Chodkowski, owner of a part [of the village], residing in Chodkowo kuchny, twenty-seven years of age, accompanied by Leon Chodkowski, age sixty-six, together with Michał Gutowski, age twenty-five, both owners of parts [of the village] residing in Chodkowo kuchny, and he showed us a child of the female sex born in Chodkowo kuchny on the eighth/twentieth day of the same month and year at the hour of six in the morning, of his wife Anna née Roman, twenty-two years of age. At Holy Baptism performed on this day this child was given the names Zuzanna Zofia, and her Godparents were Tomasz Niesłosemski and Kunegunda Chodkowska. This document was read aloud to the declarants and witnesses and signed by the declarants; the witnesses do not know how to write.
