Konstanty Chodkowski, son of Kacper Chodkowski and Aleksandra Milewska, was born on 16 July 1868 in Umiemy, Ciechanów Powiat, Płock Gubernia, Congress Kingdom of Poland. She was baptized in the parish church in Zielona, Ciechanów Powiat, Płock Gubernia, Congress Kingdom of Poland on 17 June 1866.

No 42 Umiemy
This happened in Zielona on the nineteenth day of July in the year one-thousand eight-hundred sixty-eight at the hour of eight in the morning. There appeared Kacper Chodkowski, a farmer, thirty-five years of age and residing in Umiemy, in the presence of Stanisław Kołakowski, forty-five years of age, and Antoni Milewski, thirty-seven years of age, residing in Umiemy, and he showed Us a child of the male sex born on the sixteenth day of July of this same year at the hour of nine in the morning of his wife Aleksandra née Milewska, thirty years of age. To this child at Holy Baptism performed on this day was given the name Konstanty, and his Godparents were Edward Chrzanowski and Klara his wife. This document was read aloud to the declarants, the father, and the witnesses and was signed by Us and the declaring father; the witnesses do not know how to write.
