This happened in the village of Mały Płock on the twenty-second day of November / fourth day of December in the year one-thousand eight-hundred fifty-four at the hour of two in the afternoon. There appeared Wojciech Gołaś, a peasant farmer residing in Kąty, forty-one years of age, in the presence of Jan Marzec, twenty-one years of age, together with Franciszek Prusinowski, thirty years of age, both farmers from Kąty, and he showed Us a child of the male sex born in Kąty yesterday at the hour of six in the evening of his wife Maryanna née Jorzak, forty years of age. To this child at Holy Baptism performed on this day was given the name Walenty, and his Godparents were Józef Chaberek and Małgorzata Rajnko. This document was read aloud to the declarant father and the witnesses who do not know how to write.
