Translated from the Polish, the record states:

Burial 4, Borze-marcisze

It happened in Gzy on the twenty-third day of January in the year one-thousand eight-hundred twenty-six at the hour of twelve noon. There appeared Stefan Bórski, age sixty-six years, and Antoni Bórski, age sixty years, both farmers residing in the village of Borze Marcisze and they stated that on the twentieth day of the present month at the hour of nine in the morning, Baltazar Bórski died, sixty years of age, leaving behind his widowed wife Katarzyna Napierkowska. After visual confirmation of the death of Baltazar, this document was read aloud to the declarants and signed below by us.
