Jan Chodkowski, son of Wojciech Chodkowski and Joanna Grabowska, was born on 28 January 1861 in Rączki, Ciechanów Powiat, Płock Gubernia, Congress Kingdom of Poland. He was baptized in the parish church in Zielona, Ciechanów Powiat, Płock Gubernia, Congress Kingdom of Poland on 28 January 1861.

Nr 4 Rączki
This happened in the village of Zielona on the twenty-eighth day of January in the year one-thousand eight-hundred sixty-one at the hour of nine in the morning .  There appeared Wojciech Chodkowski, twenty-one years of age, residing in Rączki, in the presence of Stanisław Żbikowski, fifty years of age, and Daniel Grabowski, fifty years of age, residing in Rączki, and he showed Us a child of the male sex, born today at the hour of six in the morning of his wife Joanna née Grabowski, twenty-nine years of age. To this child at Holy Baptism performed on this day was given the name Jan, and his Godparents were Stanisław Żbikowski and Józefa Żbikowska. This document was read aloud to the declarant father and the witnesses and was signed by Us. The declarant and witnesses do not know how to write.
